Forms & Fees

Important information regarding consultations, required forms, and appointment policies.

Required Forms

The service waiver form, questionnaire applicable to the species of your pet, 3-5 short 3 minute or less videos of your pet not placing any pet or person in an unsafe situation, previous medical records, and RDVM teleconsulting form, if applicable, to the type of appointment scheduled need to be filled out and returned 72 BUSINESS hours prior to your appointment. 

 

If you have scheduled and reserved an appointment time but do not have your forms in by the required time and then your appointment needs to be rescheduled or moved, you will be subject to the cancellation policy and forfeit your deposit which is the cost of the appointment scheduled. 

Fees: In-Person or Teleconsulting

  • Initial Behavior Consultation 

    • $450/ up to 1 hr 20 min - Equine, Canine, Avian (pet), and Feline 

  • Initial Behavior Consultation  

    • $250 ( 40min-1 hour)- Porcine, Caprine, Small Mammals, Chicken, Wildlife 

  • Progress Assessment Consultation

    • $225 - 40 minutes of time - Equine, Canine, Avian, Feline 

  • Initial Behavior Consultation 501c3 or Zoo

    • $300 / 1 hr - MUST HAVE A DVM ON STAFF  and TELECONSULTING FORM SUBMITTED BY A DVM if electing for virtual option

Veterinary-to-Veterinary Consultations

  • $175/flat rate paid for by the veterinary clinic or client requesting a consultation for their pet with the pet's primary care veterinarian.

  • 30 min phone call.

  • History, medical records, and video pertinent to the case will be reviewed at the time of the consultation. Please email info@veterinarypsychiatry.com for scheduling options and a detailed handout on this option of care. 

  • The phone consultation will not include a written report and the attending veterinarian will make all final decisions on implementing recommendations given by the consulting veterinarian. 

Cancellation Policy

Behavior consultations with veterinarians with a special interest in behavior and board-certified veterinary behaviorists are in high demand. We try to help as many pets as possible and with that can come long wait times for some pet parents. We do not double book appointments as the entire consultation time is spent with the client and patient during their allotted time frame. Therefore, late cancellations and no-shows can take valuable time that could have been utilized to help another client and pet in need.

We understand that circumstances may arise or come up unexpectedly that create a situation where you are unable to keep your pet's scheduled appointment. If your pet's appointment is rescheduled BEFORE the 72 business hour window of your pet's appointment then you do not lose your appointment fee and it is applied to your next scheduled appointment within a 3-month time period. This is allowed one time for each newly scheduled appointment, and additional needs to reschedule the same appointment within 72 business hours will result in the loss of your prepaid appointment fee and an additional appointment fee will be required to schedule. â€‹

If you cancel your appointment with less than 72 business hours notice of your appointment, then you will forfeit half of your appointment fee for compensation for liquidated damages. Your pets videos, medical history, and questionnaire have already been reviewed at this point in time and behavior consultations cannot be readily filled in less than 72 hours unlike general practice or emergency medicine due to a variety of reasons.  This policy applies to appointments that must be rescheduled due to a client's excessively late arrival (20 minutes or greater) 'whether it be in person or telemedicine'. Scheduling an appointment for your pet whether it be in person or telemedicine means you understand and agree to these terms. Should you need to cancel or reschedule an appointment within the allotted time frame, then please notify via info@veterinarypsychiatry.com as we need this in writing to be filed under your pet’s patients chart in which you will receive a reply. It is your responsibility to verify that the cancellation information has been received.
