Animal behavior disorders can be challenging for all people and animals involved and continued behavioral progress assessments, frequent communication, and client participation is needed to assess whether your pet will improve and to the degree your pet will improve. It is common for many psychiatric diagnoses and/or behavior concerns to improve but it is also important to note they are not curable. Behaviors by your pet such as but not limited to barking, snarling, growling, lunging, kicking, snapping, biting, hissing, scratching may still occur at some point in future even with treatment. You could be held solely legally and financially responsible for any injuries that your pet causes to another person or animal. You alone are responsible for avoiding situations that could result in injury to you, another person, your pet, or another animal. By signing this form you are indicating you understand that managing behavior problems involves inherent risk, and that you agree not to hold Veterinary Psychiatric and Behavior Center PLLC, Dr. Heather Tucker, or any other person or entity employed by or affiliated with the preceding persons or entities responsible for any injuries that may happen to you, other people, other animals or your pet during the course of treatment of your pet or following the course of treatment of your pet.
